📋 Description

• Provide community-based care to vulnerable seniors.
• Build long-term relationships with fewer patients.
• Lead in-person assessments with the IDT to create care plans.
• Coordinate discharge for various placements.
• Focus on comprehensive care; no hospital rounds; ~8 patients/day.

🎯 Requirements

• ACGME-accredited residency completed.
• Master's or Doctorate degree required.
• Board certification preferred.
• Active US state medical license without restrictions.
• CDS certification and DEA registration.
• Eligible for Medicare/Medi-Cal enrollment.
