Speech Language Pathologist, SLP, Senior Living Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to diagnose and treat speech, language, or swallowing issues.
• Develop a treatment plan based on evaluation, physician orders, and goals.
• Continually assess and revise care plans with the multidisciplinary team.
• Educate patients and families in rehabilitative care to promote independence.
• Document observations and care using a state-of-the-art tablet.

🎯 Requirements

• Current PA license as Speech Language Pathologist.
• ASHA CCC-SLP or equivalent certification.
• Meet certification requirements; may await ASHA exam.
• Complete education and supervised work for certification.
• Solid computer skills; EMR experience preferred.
