📋 Description

• Lead, inspire and motivate others to provide exceptional care.
• Direct care coordination by a multi-disciplinary home health team.
• Process incoming referrals.
• Build relationships with referral sources, physicians, hospitals, and partners.
• Manage and mentor Clinical Associates and field staff.
• Facilitate new hire orientation for field staff.

🎯 Requirements

• RN with 2+ years of RN experience.
• Medicare Certified Home Care experience.
• Knowledge of Medicare Conditions of Participation.
• OASIS documentation and ICD-10 coding.
• Strong management and leadership background.
• Excellent organizational, interpersonal and communication skills.
