Outpatient Occupational Therapist, (OT) - Home Health

📋 Description

• Home care OT on a multi-disciplinary team to keep clients at home.
• Perform outpatient therapy evaluations to create plans of care.
• Use research-based care to maximize functional independence.
• Coordinate with home health therapy to transition to outpatient.
• Educate clients, families, and caregivers on care plans.
• Document evaluations and treatments in outpatient EMR.

🎯 Requirements

• Current North Carolina Occupational Therapist license.
• Home care experience preferred, not required.
• At least one year of OT experience.
• Graduation from an accredited OT program (AMA/AOTA).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
