Occupational Therapist, OT, Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status: muscle function, endurance, ADL/IADL.
• Assess cognitive, psychological and motor performance; develop intervention plans.
• Evaluate home environment for hazards; plan adaptations.
• Educate patients and families in rehabilitative care and activities.
• Document observations and interventions on a touch pad tablet.

🎯 Requirements

• Current OH Occupational Therapist license.
• Minimum 1 year OT experience; home health preferred.
• Degree in Occupational Therapy from AMA/AOTA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
