  • Make home visits to clients in designated territories.
  • Evaluate functional status: muscle function, endurance, coordination, ADL/IADL.
  • Assess cognitive, psychological, and motor performance; develop OT plans.
  • Evaluate home environment for hazards or barriers to independence.
  • Identify equipment needs and environmental adaptations.
  • Update OT care plan and coordinate with nursing.
  • Educate patients, families, or caregivers in rehabilitative care.
  • Document observations and interventions on a touchpad tablet.

🎯 Requirements

  • A current MD Occupational Therapist license.
  • Minimum one year of OT experience.
  • Graduation from AMA/AOTA-accredited OT program.
  • Ability to work independently and manage time.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; EMR experience preferred.
  • Prior home care experience preferred, but not required.
