- Serve as internal expert on waiver onboarding; guide orientation and onboarding.
- Perform quarterly in-home assessments for assigned cases.
- Serve as on-call backup for emergent patient situations.
- Escalate health and caregiver issues to leadership.
- Perform additional home visits as needed based on care team notes.
π― Requirements
- Bachelor 's degree or equivalent work experience.
- Must have a vehicle to travel daily to patient homes.
- 1-3 years of medical field or case management experience; updating notes.
- 2+ years of call center, telephonic outreach, or customer service experience.
- 2+ years of experience with frail or elderly individuals.
- Strong verbal and written communication; ability to establish rapport with members and caregivers.
- Excellent time management, critical thinking, and active listening.
- Strong problem-solving skills; able to identify issues and escalate.
- Ability to exercise independent judgment and voice issues or trends.
- Maintains patient confidentiality; adheres to HIPAA guidelines.
- Strong proficiency with computer software navigation and mobile apps.
- Nice-to-have: bilingual in Spanish or other languages.
