📋 Description

• Coordinate care across disciplines for the Compassionate Care Program.
• Provide prompt, compassionate patient service via an integrated tech platform.
• Support goals of care conversations and advanced care planning.
• Facilitate transitions of care and uphold patient wishes across settings.
• Communicate updates to community partners and medical practices.
• Document interactions accurately and timely per policy.

🎯 Requirements

• 3 years of hospice, palliative care, or care coordination experience.
• Team-based care experience.
• Strong knowledge of community resources.
• Excellent communication and teaching skills.
• MS Office Suite including Outlook and Teams.
• Ability to work remotely with a dedicated, interruption-free space.
