Physical Therapist- Senior Living Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility tests and measurements (ROM, strength, balance).
• Implement wound care and topical therapies as needed.
• Develop and adjust individualized PT care plans.
• Reassess care plan with nursing in the multidisciplinary team.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions using EMR/tablet.

🎯 Requirements

• Current PA Physical Therapist license.
• At least one year of recent PT experience.
• Graduation from APTA/CAAHEA-approved program (transcript/diploma).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
