📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, strength, balance, and safety.
• Implement wound prevention and care using evidence-based dressings.
• Develop individualized care plans; include gait training and balance work.
• Update care plans with nursing in the multidisciplinary team.
• Educate clients and families on rehabilitation and safety.

🎯 Requirements

• NC Physical Therapist license.
• Graduation from APTA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
