- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements of mobility, ROM, transfer status, stability, ambulation, strength, endurance, balance, and safety.
- Implement wound care with evidence-based dressings and topical agents to prevent wounds and promote repair.
- Develop and implement individualized care plans, including manual therapy, gait training, balance, and other interventions.
- Continually assess and revise the PT care plan and collaborate with nursing in the multidisciplinary care plan.
- Educate and instruct clients and families in rehabilitative care to promote health, safety, and independence.
๐ฏ Requirements
- A current NJ Physical Therapist - PT license.
- Minimum of one year of PT experience.
- Grad from APTA/CAAHEA-accredited program; transcripts/diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
- Solid computer skills; EMR experience preferred.

- Base pay $65.00-$75.00 per point.
