Wound Care RN

📋 Description

• Conduct home health nursing assessments to determine equipment needs.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document nursing interventions per care plans.
• Maintain a healthy and safe home environment and respond to physician orders.
• Instruct participants and families on disease processes and self-care.
• Track scheduling with the Home Health Manager and coordinate transitions.

🎯 Requirements

• Graduate of an accredited school of nursing.
• Unencumbered RN license required.
• Minimum one year of nursing experience with frail/elderly population; home health preferred.
• Reliable means of transportation.
• CPR and First Aid certification within 180 days of hire; maintain thereafter.
