Conduct home health nursing assessments to determine equipment needs
Participate in IDT meetings to develop participant care plans
Deliver and document nursing interventions per care plans
Instruct participants and families on disease processes and self-care techniques
Track home health scheduling with the Home Health Manager
Collaborate with physicians and IDT for care transitions

🎯 Requirements

Graduate of an accredited school of nursing
Unencumbered RN license required
Minimum 1 year nursing in clinical settings with frail/elderly; HH experience preferred
Reliable means of transportation
CPR and First Aid certification within 180 days of hire; maintain certification thereafter
