Conduct home health nursing assessments to identify equipment needs, participant goals
Actively participate in IDT meetings to develop participant care plans
Deliver and document home health nursing interventions per care plans
Instruct patients, families, and caregivers on disease, self-care and prevention
Track home health scheduling with the Home Health Manager or scheduler
Collaborate with physician and IDT for smooth care transitions

🎯 Requirements

Graduate of an accredited school of nursing
Unencumbered RN license
1+ year nursing experience with frail/elderly; home health preferred
Reliable transportation
CPR and First Aid certification within 180 days of hire; certification maintained thereafter
CPR and First Aid certification on hire preferred
