Conduct home health nursing assessments to determine equipment needs and goals.
Participate in IDT meetings to develop participant care plans.
Deliver and document nursing interventions per care plans, meds, tests, treatments.
Instruct participants, families, and caregivers on disease and self-care.
Track and monitor scheduling with the Home Health Manager.
Collaborate with the physician and IDT for smooth transitions between settings.

🎯 Requirements

Graduate of an accredited school of nursing.
Unencumbered RN license required.
Minimum 1 year nursing experience with frail/elderly; home health preferred.
Reliable means of transportation.
CPR and First Aid certification within 180 days of hire; preferred at hire.
