Primary Care Provider, Psychiatric-Mental Health (NP/PA)

📋 Description

• Provide psychiatric consultation, evaluation, counseling, and treatment with PACE physicians.
• Perform in-person or telehealth psychiatric assessments and develop a care plan with the IDT.
• Collaborate with physicians and IDT in discharge planning from acute or long-term care.
• Refer participants to psychiatrists as indicated in consultation with PACE physicians.
• Oversee care of participants in the nursing home, making regular visits as required.
• Be a consultant to the IDT, communicating participant changes and coordinating 24-hour care.

🎯 Requirements

• Master’s Degree in Nursing or Physician Assistant with PMH emphasis.
• Certified as PMH Nurse Practitioner or Physician Assistant.
• Active PMH NP/PA license; NPI; CDS certification and DEA license.
• State license without restrictions.
• Reliable transportation.
• Minimum 2 years of experience with the frail or elderly.
