Conduct Home Health Nursing Assessments and participate in IDT to develop care plans
Deliver and document nursing interventions per care plans; meds and tests
Instruct patients and families on disease process, self-care, and prevention
Track and monitor Home Health scheduling with HH Manager
Collaborate with physician and IDT for smooth care transitions

🎯 Requirements

Graduate of an Accredited School of Nursing
Unencumbered RN License required
1 year nursing experience in clinical setting with frail/elderly; home health preferred
Reliable transportation
