Track and monitor Home Health scheduling with the manager or contact.
Collaborate with physicians and IDT to ensure smooth care transitions.

🎯 Requirements

Graduate of an accredited School of Nursing.
Unencumbered RN license required.
Minimum 1 year nursing experience with frail/elderly; home health preferred.
Reliable transportation.
