- Provide hands-on personal care and assistance with ADLs in participants' homes
- Visit 2-5 patients per day per care plan
- Reimburse travel time and mileage between participants
- Escort participants to medical appointments and assist transfers
- Support home exercise programs per IDT care plan
- Provide translation services as needed
🎯 Requirements
- High school diploma or equivalent
- Current BLS/First Aid certification required
- CNA or HHA certification preferred
- Bilingual preferred
- Registered with Home Care Registry prior to start date
