- Provide hands-on personal care and assistance with ADLs at participants' homes
- Assist with Instrumental ADLs: laundry, light housekeeping, shopping, meals
- Measure and document vital signs as instructed
- Act as medical escort to appointments and assist with safe transfers
- Support Rehabilitation team with home exercise programs per care plan
- Provide translation services as needed
🎯 Requirements
- High school diploma or equivalency
- Current BLS/First Aid Certification
- CNA or HHA certification preferred
- Bilingual preferred
- Must be registered with Home Care Registry prior to start date
