- Follow designated care plans per patient needs.

- Case management and coordination with Clinical or Client Services.
🎯 Requirements
- Current RN license in Massachusetts.
- Minimum one year of recent clinical (medical/surgical) experience.
- Prior home health / OASIS documentation experience strongly preferred.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills; solid computer skills; EMR preferred.
- Willingness to travel to cases as assigned.
