Perform adult/geriatric home visits per plan in Scotland, Moore, Richmond, Robeson counties.
Follow care plans; administer meds, wound care, treatments.
Document observations and interventions using a tablet EMR.
Travel to cases as assigned; coordinate with Clinical/Client Services Manager as needed.
Case management and coordination within home health services.

🎯 Requirements

Current NC RN license.
Minimum 1 year clinical (medical/surgical) exp.
Home health/OASIS documentation experience required.
Home care experience preferred.
Ability to travel to cases; work independently and manage time.
Strong interpersonal skills; EMR experience preferred.
