Weekend Physical Therapist, PT, Senior Living Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, ROM, transfer status, strength and balance assessments.
• Implement wound care and integumentary interventions using evidence-based dressings.
• Develop and update individualized care plans, manual therapy, gait and balance training.
• Document observations and interventions in EMR; collaborate with nursing.
• Educate clients and families to promote health, safety, and independence.

🎯 Requirements

• Current DE PT license.
• Minimum of one year of recent PT experience.
• Graduation from an APTA/AMA-accredited PT program (as shown by transcript/diploma).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
