Weekend Physical Therapist, PT, Senior Living Visits

📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, transfer status, balance, strength, safety.
• Implement wound care with evidence-based dressings.
• Develop individualized care plans: manual therapy, gait training, balance.
• Continuously assess and revise PT care plan with nursing.
• Educate clients and families to promote health and independence.

🎯 Requirements

• Active DE Physical Therapist license (PT).
• Minimum one year of recent PT experience, home health preferred.
• Graduation from APTA-approved or CAAHEA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
