Make home visits to clients in designated territories.
Assess mobility, ROM, transfers, balance, strength, and safety.
Implement wound care with evidence-based dressings and topical agents.
Develop/implement personalized care plans with manual therapy and gait training.
Continuously assess and revise PT care plans with nursing.
Educate clients and family members in rehabilitative care and independent living.

🎯 Requirements

Current CO PT license.
Minimum 1 year PT experience.
Graduate from an APTA-approved or AMA CAAHEA-accredited program.
Ability to work independently and manage time.
Strong interpersonal, organizational, and problem-solving skills.
Solid computer skills; EMR experience preferred.
