Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements (mobility, ROM, transfer status, ambulation, strength, endurance, balance, safety).
Implement wound care with evidence-based dressings and topical agents.
Develop and implement individualized care plans (manual therapy, gait training, balance, other interventions).
Continually assess and revise the PT care plan and participate with nursing in the multidisciplinary care plan.
Educate clients, family members, or other representatives in rehabilitative care to promote health, safety, and independence.
