Making visits to clients in designated senior living communities
Perform mobility and safety assessments (ROM, strength, balance)
Develop and implement individualized care plans, including manual therapy and gait training
Continually assess and revise the PT care plan and participate with nursing in the multidisciplinary care plan
Educate clients and families to promote health, safety and independence
Accurately document observations and interventions on a touch-pad tablet

🎯 Requirements

Current Massachusetts Physical Therapist license
Minimum one year of clinical experience as a licensed PT
Home Health experience is required
