Weekend Baylor Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility/ROM, gait, strength, balance, and safety assessments.
• Provide wound care and dressings using evidence-based methods.
• Develop and implement individualized care plans with manual therapy and gait training.
• Continually assess and revise the PT care plan with nursing in multidisciplinary care.
• Educate clients and family members in rehabilitative care for health and independence.

🎯 Requirements

• PA Physical Therapist license.
• Minimum one year of PT experience.
• Graduate from an APTA/AMA-approved PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
