Make home visits for Start of Care (Admissions) in designated territories.
Assess mobility, ROM, gait, strength, endurance, balance, safety.
Develop and implement individualized PT care plans with manual therapy, gait training, balance.
Continuously assess and revise PT plan; coordinate with nursing in the multidisciplinary team.
Educate clients and families in rehabilitative care for health, safety, and independence.
Document observations and interventions using a touch pad tablet.

🎯 Requirements

A current New Jersey Physical Therapist License.
A minimum of one year of recent work experience as a Physical Therapist.
OASIS experience required and HCHB strongly preferred.
