Maintain a caseload with complex Behavioral Health cases.
Work with a multidisciplinary team and participate in case management meetings.
Complete initial assessments and determine appropriate level of care.
Conduct workshops, trainings, and provide educational resources as the clinical expert.
Review records and coordinate with referral sources, families, and outside systems as needed.
Prepare and update treatment plans in collaboration with the case manager.

🎯 Requirements

Valid, unrestricted license in Tennessee as a LCSW or LPC/MHSP
Masters or Doctorate Degree in Social Work or a similar field
Demonstrated knowledge working with multidisciplinary teams
Experience working with or serving veteran populations is preferred
