  • Own care quality and outcomes for assigned participant panel; collaborate with PCP.
  • Conduct PACE nursing assessments every 6 months; educate on conditions; follow-ups (virtual/in-person).
  • Develop discipline-specific POC with IDT; ensure holistic care; document in EHR.
  • Coordinate care for high-complexity participants across hospital, ER, and facilities; ensure seamless transitions.
  • Act as triage lead to support ER diversion via walk-in clinics and clinical escalations.
  • Dispense medications and reinforce education to participants.
  • Coordinate care on behalf of participants with other health care personnel; follow up on care measures.

🎯 Requirements

  • Graduate of an accredited school of nursing
  • Unencumbered RN license
  • Minimum 2 years nursing experience with frail/elderly population
  • Self-motivated, able to work in changing environments; make quick decisions in emergencies
  • Excellent clinical, organizational, and communication skills with seniors, families, and IDT
  • Willing to travel up to 100% of the time; multi-state support; travel expenses covered
  • Willing to work evenings, nights, weekends, and overtime
  • Must obtain CPR and First Aid within 180 days; certification preferred
