- Coordinate care for high-complexity participants across hospital, ER, and facilities; ensure seamless transitions.
- Act as triage lead to support ER diversion via walk-in clinics and clinical escalations.
- Dispense medications and reinforce education to participants.
- Coordinate care on behalf of participants with other health care personnel; follow up on care measures.
π― Requirements
- Graduate of an accredited school of nursing
- Unencumbered RN license
- Minimum 2 years nursing experience with frail/elderly population
- Self-motivated, able to work in changing environments; make quick decisions in emergencies
- Excellent clinical, organizational, and communication skills with seniors, families, and IDT
- Willing to travel up to 100% of the time; multi-state support; travel expenses covered
- Willing to work evenings, nights, weekends, and overtime
- Must obtain CPR and First Aid within 180 days; certification preferred
