Own care quality and outcomes for assigned participants; collaborate with PCP.
Conduct PACE nursing assessments (min every 6 months) and follow up in person or virtually.
Develop and implement a discipline-specific POC with IDT for holistic care.
Coordinate care for high-complexity participants across hospital, ER, and facilities.
Act as triage point to maximize ER diversion via walk-in clinic management.
Dispense medications and reinforce education for participants.

Salary: $82.55/hr + Bonus
