Traveling Primary Care Provider (NP/PA)

📋 Description

• Provide primary care to seniors in a value-based PACE program.
• Build long-term relationships with a smaller patient panel.
• Collaborate within an interdisciplinary team for holistic care.
• Travel to California clinics; include home visits as needed.
• Regular hours 8am-5pm, Mon-Fri; rotating on-call schedule by phone.

🎯 Requirements

• Licensed Nurse Practitioner or Physician Assistant with 2+ years elderly care experience.
• Prescriptive authority and DEA registration.
• Eligible to serve Medicare/Medi-Cal.
• CPR and First Aid certification within 180 days of hire (certification on hire preferred).
• Ability to travel to California clinics (travel required).
