Travel Nurse Provider, Nurse Practitioner / Physician Assistant

📋 Description

• Provide primary care to seniors in WelbeHealth's PACE program.
• Travel to California clinics and perform home visits.
• Build long-term patient relationships with a smaller caseload.
• Collaborate with an interdisciplinary team for holistic care.
• Maintain prescriptive authority, DEA registration, and CPR/First Aid certifications.
• On-call a few nights or weekends by phone.

🎯 Requirements

• Licensed Nurse Practitioner or Physician Assistant.
• 2+ years experience with elderly population.
• Prescriptive authority with active DEA registration.
• CPR and First Aid certification due within 180 days of hire.
• Eligible for Medicare/Medi-Cal participation.
• Willing to travel to multiple California clinics.
