🎯 Requirements
- Current license in good standing as RN, LPN, MSW, PT or OT.
- Demonstrated strong interpersonal skills.
- Knowledge of Medicare COPs and program requirements.
- At least 1 year experience in health care, preferably home health care.
- Previous discharge planning experience preferred.
