📋 Description

• Build relationships with referral sources and hospital contacts.
• Educate leaders about BAYADA services and home health care.
• Use clinical knowledge to explain eligibility and expectations.
• Manage referral intake and coordinate transition to home care.
• Market BAYADA services and articulate features/benefits.
• Coordinate with service office for ordered services; weekly referral log.

🎯 Requirements

• Bachelor’s Degree.
• Demonstrated interpersonal skills.
• Knowledge of Medicare COPs and program requirements.
• At least one year experience in home health care.
• Previous discharge planning experience preferred.
• Sales/Business Development background a plus.
