Transitional Care Manager, Home Health

📋 Description

• Build relationships with referral sources and hospital contacts.
• Educate leaders about BAYADA services and home health benefits.
• Use clinical knowledge to explain eligibility and expectations.
• Manage referral intake and initial transition to home care.
• Hybrid role with travel 3 days/week in Bristol County.

🎯 Requirements

• Strong interpersonal skills.
• Knowledge of Medicare COPs and program requirements.
• 1+ year in health care, preferably home health.
• Discharge planning experience preferred.
• Current license in good standing (RN, LPN, MSW, PT, OT) in state.
• Sales/Business Development background a plus.
