🎯 Requirements
- Current license in good standing as RN, LPN, MSW, PT or OT
- Bachelor’s degree preferred
- Strong interpersonal skills
- Knowledge of Medicare COPs and program requirements
- At least 1 year experience in health care, preferably home health
- Previous discharge planning experience preferred
