Transitional Care Manager, Home Health

📋 Description

• Build relationships with hospital and facility contacts to educate leaders.
• Educate patients on home health eligibility and expectations.
• Manage referral intake and initial transition to home care.
• Coordinate with the service office to set up physician-ordered services.

🎯 Requirements

• Current license in good standing (RN/LPN/MSW/PT/OT) preferred, not required.
• Strong interpersonal skills.
• Knowledge of Medicare COPs and program requirements.
• At least 1 year in health care, preferably home health.
• Discharge planning experience preferred.
• Ability to educate providers, patients, and families.
