Transitional Care Director, Private Duty Nursing

📋 Description

• Lead design, development, and management of the TCP Program.
• Train transitional care managers, families, and clients per the TCP Playbook.
• Manage TCP metrics and program data.
• Develop TCP marketing via market research and planning.
• Build relationships with key referral sources.
• Represent BAYADA at educational events and promote programs.

🎯 Requirements

• Four-year degree in nursing, marketing or related field.
• Formal sales training; ability to develop a sales/marketing plan.
• Licensed Registered Nurse preferred.
• Experience in nursing, discharge planning or home care with high-tech/complex care.
• Two+ years of supervisory/management experience in health care.
• Strong interpersonal and communication skills.
• Ability to read, write and communicate in English.
