Transitional Care Director, Home Care

📋 Description

• Demonstrate and communicate The BAYADA Way: compassion, excellence, reliability.
• Lead and support TCP program design, development, and management.
• Train transitional care managers, families and clients per the TCP Playbook.
• Responsible for TCP metrics management.
• Develop TCP program through promotion, market research and planning.
• Participate in projects that support the TCP program.

🎯 Requirements

• Four-year college degree in nursing, marketing or related field.
• Formal sales training; ability to develop and implement a sales/marketing plan; ops exp may substitute.
• Licensed Registered Nurse, strongly preferred.
• Experience in nursing, discharge planning or home care with high-tech and complex care.
• Two years' supervisory/management experience in health care.
