Telehealth Triage RN

📋 Description

• Work fully remote; not on call.
• Build relationships with participants, not just short-term care.
• Coordinate telehealth meetings between participants and clinicians.
• Ensure timely care delivery; escalate issues as needed.
• Troubleshoot logistics for transport, meds, and discharge when day teams unavailable.
• Support smooth care transitions between settings; escalate changes in participants' conditions.

🎯 Requirements

• Graduate of an Accredited School of Nursing with an unencumbered RN license.
• Nursing knowledge to treat frail elderly and manage complex clinical situations.
• Highly motivated, self-directed; adapt to changing environments and emergencies.
• Excellent clinical, organizational, and communication skills with seniors, families, and teams.
• Able to work shifts including days, evenings, nights, weekends, holidays, and overtime.
• Triaging experience preferred.
