- Support transitions between hospitals, SNFs, and home settings
๐ฏ Requirements
- RN license in good standing from an accredited program
- Nursing knowledge to treat frail elderly and manage complex cases
- Highly motivated, self-directed, adaptable to change; decisive in emergencies
- Excellent clinical, organizational, and communication skills with seniors, families, and teams
- Able to work days/evenings/nights/weekends/holidays/overtime
- Triaging experience preferred
