π― Requirements
- Masterβs degree in Social Work.
- 3+ years Health Home Care Management experience.
- 3+ years supervisory experience leading clinicians/CMs.
- Strong knowledge of Health Home NYS requirements and Lead policies.
- Leadership, coaching, and performance management skills.
- Travel to field sites and respond to escalations.
- Proficiency with Netsmart, Foothold, Salesforce, UAS-NY.
- Bilingual Spanish preferred.
