  • Oversee 1-3 Case Managers; manage shifts across SF neighborhoods.
  • Lead hotline, relocations, and asset control for client supplies.
  • Outreach, intake, and assessment for case management.
  • Develop care plans; coordinate wrap-around services, shelter, housing.
  • Transport clients to appointments; connect to health and housing services.
  • Ensure accurate documentation and KPI reporting.

🎯 Requirements

  • Bachelor's degree in social work/public health, or 2+ years in outreach/case management.
  • Experience in de-escalation, motivational interviewing, overdose reversal.
  • Experience with data collection and documenting case notes.
  • Knowledge of stabilization, shelter, housing, and SUD treatment centers.
  • Must have valid California driver's license.
  • Auto insurance and clean DMV record to drive GLIDE van.
