📋 Description

• Make home visits to clients in designated geographic territories.
• Assess mobility, ROM, transfers, balance, strength, and safety.
• Implement wound care interventions using evidence-based dressings.
• Develop individualized care plans with manual therapy, gait training, balance.
• Continually assess and revise the PT care plan with nursing input.
• Educate clients and families to promote health, safety, and independence.

🎯 Requirements

• Current NJ Physical Therapist (PT) license.
• Minimum of one year of PT experience.
• Graduate from an APTA- or CAAHEA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
