Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements: mobility, ROM, strength, balance.
Implement wound prevention and healing using evidence-based dressings and agents.
Develop individualized care plans, manual therapy, gait training, balance.
Continually assess and revise care plans with nursing in the multidisciplinary team.
Accurately document observations and interventions using a touchpad tablet.

🎯 Requirements

A NJ PT license.
At least 1 year PT experience.
Graduate from APTA/CAAHEA-accredited program.
Able to work independently and manage time effectively.
Strong interpersonal, organizational, and problem-solving skills.
Solid computer skills; EMR experience preferred.
