- Make home visits to clients in designated geographic territories.
- Perform evaluations to assess and treat speech, language, or swallowing issues.
- Develop a treatment plan based on evaluation, physician’s orders, and goals.

- Document patient observations and care using a tablet.
🎯 Requirements
- Current Colorado SLP license.
- ASHA CCC-SLP or eligible and awaiting exam.
- One year of SLP experience; home health a plus.
- Solid computer skills; EMR experience preferred.
- Complete the necessary education/work requirements for certification.
- Ability to work in home health and collaborate with a multi-disciplinary team.
