Speech Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to assess, diagnose, and provide therapeutic interventions for clients with speech, language, communication, or swallowing difficulties.
• Develop a treatment plan based on the evaluation, physician’s orders, and goals.
• Continually assess and revise the speech therapy care plan with BAYADA clinicians in the multidisciplinary care team.
• Educate patients, family members, or other patient representatives in rehabilitative care to promote the client’s health, safety, and independent living.
• Document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.

🎯 Requirements

• Colorado license as a Speech Language Pathologist (SLP).
• Certificate of Clinical Competence in Speech Language Pathology (CCC-SLP) from ASHA, or equivalent, or pursuing certification.
• Educational requirements and supervised work experience for certification.
• Solid computer skills; EMR experience preferred.
• One year of SLP experience; home health experience a plus.
