Make home visits to clients in designated territories.
Perform evaluations to assess, diagnose, and treat speech, language, or swallowing issues.
Develop a treatment plan based on evaluation, orders, and goals.
Update care plans; join multidisciplinary team as needed.
Educate patients and families in rehabilitative care for independence.
Document observations and interventions using a touch-pad tablet.

🎯 Requirements

Current PA Speech Language Pathologist license.
ASHA CCC-SLP (or equivalent) certification.
Education/experience for certification or awaiting exam.
Completed education and supervised experience for certification.
Solid computer skills; EMR experience preferred.
